General Clinic Polices
We accept SELF referring patients as well as patients through referring physician or other health care facilities. Patient should be aware of their insurance policy with regards to referral requirement, if any. We are not responsible for confirming pre-approvals from insurance companies for the initial appointment.
Appointments
We advice you arrive 15 minutes before the appointment for new patients. Incase you can not keep the appointment, you need to notify office 24 hours in prior or on previous business day.
Payments
We ask for payment at the time of your visit, unless other arrangements are made. We can accept payment in the form of cash, personal checks, and credit cards. In the case of checks or credit cards, we may need to confirm your identification using, for example, a picture driver license.
